PIQ accuracy: what SIRE 2.0 inspectors ask and find
The pre-inspection questionnaire has to match the ship as she is on the day, including photos, so every change on board needs to reach the PIQ.
Do this before the inspector arrives: Compare PIQ photos and data with the ship as she is today.
What the inspector wants to see
- PIQ photos and data updated after any change
- Photos date-stamped, taken with the intrinsically safe camera
Questions inspectors ask about PIQ accuracy
What makes an acceptable photo for the SIRE 2.0 photo upload?
It must show the true current condition, be representative of the area and free of shadows. Use close deck views, show access in the steering gear room view, remove the ETA canvas cover.
Why must the HVPQ be checked thoroughly before an inspection?
The compiled VIQ (CVIQ) is generated largely from HVPQ data, so any HVPQ error produces wrong questions in the inspection.
From when does the Estimated Index Value (EIV) apply, as asked in HVPQ 1.2.4?
From 1 January 2023, for vessels that do not have an EEXI.
What mooring drum details go into the HVPQ?
Details of every mooring drum. The SBM mooring line storage drum has a larger diameter than the others.
Can you upload a photo taken on a sister ship?
No. A photo from another vessel was flagged because it did not show the double block and bleed valve fitted on board.
If HVPQ 5.2.1.1 (helicopter operations) is answered 'yes', what must be in place?
Helicopter RA, training/drill requirements, landing area plan, ICS helicopter checklist, assigned officer and crew, emergency equipment, limits on cargo/crane work.

Real PIQ accuracy observations, and how to avoid them
The photo uploaded to the OCIMF repository did not give a clear view of the main engine.
Avoid it: Review photo uploads against each required view before submitting and retake unclear images.
No photograph had been provided of the double block and bleed arrangement for the nitrogen generator.
Avoid it: Check every required photo is uploaded before the inspection.
The uploaded photo showed the port side instead of the starboard side requested; the condition itself was not a concern.
Avoid it: Double-check side and location labels before uploading inspection photos.
The uploaded photo showed the starboard side instead of the port side requested; the condition itself was not a concern.
Avoid it: Label photos by side and area when taking them to avoid upload mix-ups.
Several HVPQ entries were wrong: CAP survey areas did not match the CAP certificate, and mooring layout, bow mooring and manifold diagrams were missing.
Avoid it: Cross-check HVPQ entries against certificates and attach all required diagrams before the inspection.
The uploaded photo showed the cargo tank's ordinary P/V devices instead of the requested item; the correct photo was taken at the inspection.
Avoid it: Check each uploaded photo matches the exact equipment named in the requirement.
HVPQ entries were out of date for the last PSC inspection, last ballast tank inspection and last winch brake test.
Avoid it: Update the HVPQ after every PSC, tank inspection and brake test, not just before vetting.
The uploaded photo showed the port manifold looking forward to aft, not the view requested.
Avoid it: Take repository photos from the exact angle and side specified.
